For screening I need either one of the following:

• Preferred411 member ID (with at least two provider okays from currently active providers that you've seen within the last year), your full name, and your cell phone number

• Two provider references from reputable, active providers that you've seen within the last year (please include their name, email, website, and when/where you visited them) and the name, phone number, and email that you used with them

• Name of employer, work email, work phone number, cell phone number, a copy of your driver's license (or passport for non US citizens), and your current city and state of residence
